Device Loan Agreement

For Students at Hednesford Valley High School

 

1. This agreement is between: 

1) Hednesford Valley High School, Stanley Road, Cannock, WS12 4JS (“the school”) 

2) ________________________________ (“the student” and “I”)

And governs the use and care of devices assigned to individual students. This agreement covers the period from the date the device is issued through to the return date of the device to the school.

 

2. Damage/loss 

By signing this agreement I agree to take full responsibility for the equipment and will look after it at all times.

If the equipment is damaged, lost or stolen I will immediately inform my parent/carer and Mr Watson.

I agree to keep the equipment in good condition.

 

3. Unacceptable use 

I am aware that the school monitors my activity on the equipment.

I will ensure any activity that my school considers ‘unacceptable use’ will not be carried out on the equipment.

I will follow our Acceptable Use Policy, and will not:

·       Access, create, store or link to or send material that is pornographic, offensive, obscene, illegal or otherwise inappropriate.

·       Share confidential information about the school, its students, or other members of the school community

·       Install or attempt to install any software, applications or web services on this device without approval by school, or create or using any programme, tool or item of software designed to interfere with the functioning of the device, accounts or data.

·       Uninstall or attempt to uninstall any software or management tools, unenrol device management, wipe or alter software.

·       Disassemble, reconfigure, upgrade or change any hardware of any device.

·       Use inappropriate or offensive language, or language that could embarrass your school.

I accept that if I do not follow the rules there will be consequences. 

This could be a ban from some school equipment, or I might have to give the equipment back.

 

 

4. Personal use 

The equipment will only be used by me and the equipment will not be given to anyone else.

I understand the equipment may be used for personal use outside of school hours; but I am aware that all activity may still be monitored by the school and that the Acceptable Use Policy still applies at all times.

 

5. Data protection 

I agree to:

·       Make sure the equipment is locked if left inactive for a period of time

·       Do not share the equipment among family or friends

I understand that the equipment will be monitored at all times and staff may be able to see posts I make online.

I accept any illegal activity may be reported to the police. 

 

6. Return date 

I will return the device in its original condition to the school office within one week of being requested to do so.

 

7. Consent   

By signing this form, I confirm that I have read and agree to the terms and conditions set out above and agree that the school can recall the equipment at any time, for any reason.
